1. How to Apply for DDA Services

To apply for services through the Developmental Disabilities Administration (DDA), contact one of the following regional offices based on your location:

  • Central Maryland Regional Office (CMRO)
    (Anne Arundel County, Baltimore City, Baltimore County, Harford County, Howard County)

  • Eastern Shore Regional Office (ESRO)
    (Caroline County, Cecil County, Dorchester County, Kent County, Queen Anne's County, Somerset County, Talbot County, Wicomico County, Worcester County)

2. Eligibility Requirements for DDA Services

An individual may qualify for a full range of DDA services if they have a severe, chronic disability that:

  • Is due to a mental and/or physical impairment.

  • Appeared before the age of 22.

  • Is expected to last indefinitely.

  • Requires ongoing external support or assistance to live independently.

  • Necessitates specialized, multi-disciplinary, or long-term services that are individually planned and coordinated.

  • Results in substantial functional limitations in at least three of the following areas:

    • Self-Care

    • Receptive & Expressive Language

    • Learning

    • Mobility

    • Self-Direction

    • Capacity for Independent Living

3. Required Documents

To apply for services, you will need to provide the following information:

  • Full name, address, and phone number

  • Date of birth

  • Social Security number

  • Insurance details (e.g., Medical Assistance, Medicare, SSDI)

  • Type of service requested (e.g., day services, residential, or support services)

  • Documentation of disability

  • Primary contact person and their phone number
